anti-hyperlipidemics Flashcards
ezetimibe AE
- low incidence of liver/skeletal muscle damage
ezetimibe MOA
cholesterol absorption inhibitor; inhibitor of NPC1L1
what are the bile acid binding resins?
cholestyramine, colestipol
What ion do the bile acid binding resins excgange
chloride ion for bile acids
which statins are not cyp substrates?
pravastatin, pitavastatin
which statins are prodrugs?
lovastatin and simvastatin
statin MOA
HMG-CoA reductase competitive inhibitors
what is HMG co-A reductase?
the rate-limiting enzyme in cholesterol biosynthesis
what intermediate in the cholesterol synthesis pathway do statins resemble?
mevalonate (mevalonic acid)/HMG CoA
which statins are metabolized by CYP 3A4?
lovastatin, simvastatin, and atorvastatin
which statins are metabolized by CYP 2c9?
Fluvastatin, rosuvastatin
which statin is excreted in the bile?
pitvastatin (avoid if liver issues?)
enterohepatic recirculation
AE of statins
- rhabdomyolysis: myopathy with renal dysfunction.
- monitor CPK (serum creatinine phosphokinase)
- increased incidence with co-adm with cyp inhibitors (gemfibrozil)
- hepatotoxicity: monitor serum transaminase
- increased incidence of T2DM
what is the ATP citrate lyase inhibitor?
bempedoic acid (nexletol)
MOA of bempedoic acid
- ATP citrate lyase inhibitor
- enzyme upstream of HMG-CoA reductase
