What is metoclopramide used for?

A

1) prophylaxis and treatment of nausea and vomiting

especially with reduced gut motility

How does metoclopramide work?

A

Metoclopramide blocks the dopamine2 receptors in the chemoreceptor zone (brain) and the GI tract.
This leads to:
- increased gastric emptying
- less stimulation of the medulla’s “vomiting centre”

What are the contraindications of metoclopramide?

A
  • avoid use in children and young adults as movement abnormalities more common.
  • GI obstruction due to risk of perforation as drug has prokinetic effect

  • Antipsychotic (movement abnormalities increased when combined)
  • dopaminergic agents of parkinsons (these drugs work against each other)

When can metoclopramide be considered for GORD?

A

In GORD treatment when PPIs and H2 blockers aren’t enough then add metoclopramide. It helps increase rate of gastric emptying.
