What is metoclopramide used for?
1) prophylaxis and treatment of nausea and vomiting
especially with reduced gut motility
How does metoclopramide work?
Metoclopramide blocks the dopamine2 receptors in the chemoreceptor zone (brain) and the GI tract.
This leads to:
- increased gastric emptying
- less stimulation of the medulla’s “vomiting centre”
what are the side effects of metoclopramide?
- diarrhoea
- ## movement abnormalities
What are the contraindications of metoclopramide?
- avoid use in children and young adults as movement abnormalities more common.
- GI obstruction due to risk of perforation as drug has prokinetic effect
What are the drug interactions of metoclopramide?
- Antipsychotic (movement abnormalities increased when combined)
- dopaminergic agents of parkinsons (these drugs work against each other)
what is the starting dose for metoclopramide?
10mg up to three times daily PO / IM / IV
When can metoclopramide be considered for GORD?
In GORD treatment when PPIs and H2 blockers aren’t enough then add metoclopramide. It helps increase rate of gastric emptying.
