Anti-Arrhythmic Agents Flashcards
Objectives
1
Q
Class 1A MOA
A
- Na+ channel blocker
- prolong conduction and re-polarization
- more rhythm controlled
2
Q
Quinidine Indication
A
- Brugada syndrome, rarely Afib and VT
3
Q
Quinidine MOA
A
- Class 1A
- Na+ channel blocker
- also blocks multiple K+ channels
- slowing of CV, prolongation of APD
4
Q
Quinidine AE
A
- toxicity-cinchonism
- blurred vision
- tinnitus
- HA
- psychosis
- QT prolongation
- thrombocytopenia
- enhanced digitalis toxicity
- anticholinergic
5
Q
Quinidine Drug Interaction
A
- inhib of CYP2D6
- Enhanced digitalis toxicity
- QT prolongation w/ azithromycin and clarithromycin
6
Q
Quinidine Black Box Warning
A
- Due to enhanced proarrhythmic effects and ability to worsen heart failure symptoms, class IA drugs should not be used in patients with atherosclerotic heart disease or systolic heart failure.
7
Q
Quinidine Monitoring
A
- low K+
- Na+
8
Q
Disopyramide (Norpace) Indication
A
- ventricular arrhythmias
- rarely in A. fib
9
Q
Disopyramide MOA
A
- Class 1A, Na+ channel blocker
- stabilizes membranes
- depresses action potential in phase 0
10
Q
Disopyramide Drug Interactions
A
- antifungals
11
Q
Disopyramide AE
A
- Anticholenergic effects (more than quinidine)
- QT prolongation
12
Q
Dysopyramide Monitoring
A
- low K+ = ineffective
- high K+ = enhanced toxicity
13
Q
Procalnamide (IV only) Indications
A
- testing for Brugada syndrome
- rarely A. fib and VT
14
Q
Procalnamide MOA
A
- Class 1A = Na+ channel blocker
- also blocks K channel;
- slowing of conduction velocity
- prolongation of action potential duration
- decreases automaticity
15
Q
Proclanamide AE
A
- nausea
- procainamide-induced lupus syndrome (more common in slow acetylators)
- hypotension
