Nonfunctional pituitary tumors classically present as
bitemporal hemianopsia
Most common functional anterior pituitary tumor
prolactinoma
women: galactorrhea and amenorrhea
men: loss of libido
GH adenoma may cause these conditions
gigantism in children
acromegaly in adults
Most common cause of death in gigantism/acromegaly
heart failure
heart and other tissues are also enlarged in this condition
Dx of GH adenoma
elevated GH and IGF-1
Endocrine complication of GH adenoma
2˚ diabetes mellitus
GH induces gluconeogenesis
Tx of GH adenoma that isn’t surgery
octreotide
SS analogue, prevents GHRH from releasing GH
Masses that may cause hypopituitarism in
adults
children
adults: pituitary adenoma
children: craniopharyngeoma
Sheehan syndrome
etiology
complications
presentation
- caused by massive hemorrhage during delivery 2˚ to fact that pituitary very large in pregnant women but blood supply does not increase at all
- causes panhypopituitarism
- presents as loss of pubic hair and decreased ability to make milk
Central diabetes insipidus etiology
ADH deficiency
hypothalamic or pituitary pathology
Common drug causing nephrogenic diabetes insipidus
¡¡¡lithium!!!
Tx of central diabetes insipidus
desmopressin
vasopressin ie ADH analogue
SIADH etiology
ectopic production, either from post pituitary or
paraneoplastic syndrome ie small cell lung cancer
also other pulmonary pathologies ie COPD
SIADH is common paraneoplastic syndome of this primary neoplasm
small cell lung cancer
Tx of SIADH
demeclocycline
