Purpose and requirements for completion of the annual report of personnel exposure to ionizing radiation (6470/1) to NDC
Provide BUMED via NDC exposures for all personnel on board 31 Dec who were monitored for exposure to ionizing radiation during the year.
Submitted to arrive prior 1 April each year
If operational commitments delay receipt of final exposure information for the previous year after 1 March, the report shall be submitted within 30 days of the date of receipt.
Following remarks must appear in the remarks section of the report if they occurred during the reporting period

For badging periods that span 1 Jan, the year in which the mid-point of the exposure period occurs shall be the year in which the entire exposure for that period
Purpose and requirements of the situational report of personnel exposure to ionizing radiation
Purpose: provide BUMED via NDC exposure information for personnel who have transferred from your command prior to December 31 who were monitored for exposure to ionizing radiation since last annual report.
Reports to NDC within 30 days of detachment or within 30 days of receipt of individual’s final exposure information.

Retention: Indefinitely
Requirements for NAVMED 6470/1 situational report of personnel exceeding radiation exposure limits
shall be forwarded for any individual who receives a TEDE in excess of the limits
forwarded within 30 days from the determination of such exposure

provide exposure information for personnel on board 31 dec who were monitored associated with NNPP during the year regardless of where they received it
summary if personnel annual exposure shall be forwarded to NAVSEA 08 in letter form, or by message, if necessary, to be received at NAVSEA 08 by 31 March each year
If all results will not be available to support arrival of a final summary. send preliminary report
