anatomical pathology: spleen Flashcards

1
Q

What is two causes of congenital abnormalities in the spleen

A

Q

What is the three categories of congestive splenomegaly & causes

A
  1. Pre-hepatic: thrombosis of extrahepatic portion of portal/splenic vein
  2. Hepatic: cirrhosis
  3. Post-hepatic: raised pressure in IVC to spleen via portal system (ascites & hepatomegaly) & decompensated right heart failure/pulmonary or tricuspid disease

Q

What is the appearance of bacterial infectious splenomegaly

A

Neutrophil accumulate in sinuses & medullary cords
Becomes soft due to proteolytic enzyme action
If extreme can cause acute septic splenitis

Q

What is apparent in storage disorder splenomegaly

A

Red pulp expansion by macrophages

17
Q

What is the three causes of splenic infarction

A
  1. Occlusion of splenic artery/branches
  2. Secondary to emboli arising from the heart
  3. Local thrombosis due sickle cell anaemia, myeloproliferative disorder & malignant infiltrates
18
Q

What is the most common cause of a ruptures spleen

A

Blunt abdominal trauma

19
Q

What is the consequence of rupture of the spleen

A

Massive life threatening intraperitoneal haemorrhage

20
Q

What is the treatment for a ruptured spleen

A

Emergency splenectomy

21
Q

What is splenic atrophy

A

Reduce in size & function of the spleen due to secondary causes

22
Q

What is the cause of splenic atrophy

A

Intestinal malabsorption or sickle cell disease

23
Q

Why does sickle cell disease cause splenic atrophy

A

Multiple splenic infarcts that causes the spleen to reduce in size & function
