Definition of anaphylaxis
Systemic reaction defined as a TWO organ system involvement
ABCS of anaphylaxis
A: edema, increase secretions, swelling of tongue and lips
B: edema in secretions, bronchospasm, hyperventilation, wheezing, hypoxia
C: vasoldation, tachycardia, hypotension
D: anxiety
E: wheels and flare (urticaria), itching
5 main causes of anaphylaxis
1) Food
2) drugs ( NSAIDS!)
3) stings ( include tic bites in ten)
4) Mixed ( exercise induced)
5) unknown ( 40%)
Organ system involvement in anaphylaxis
1) skin
2) mucus membranes
3) lungs
4) heart
5) GI
Treatment of Anaphylaxis
1) EPI .3 IM, if 5 minutes w/o response give second dose. Give in thigh in ER
2) Benadryl 25 mg IV, IM, or PO
3) H2 blocker: synergystic with H1 blockers , works on bradykinin
4) steroids: 60 pred or 125 solumedrol ( blocks second wave phenomenon)
5) IV fluids 20 cc/kg ( 1 L bolus)
Dose of Epi in Anaphylaxis?
.3 IM: .3mg 1:1000 dilution
Dose of Epi in childhood anaphylaxis?
.01 mg/kg to max of .3
