2nd and 3rd degree
Hemorrhoids degree:
bleed but do not prolapse through the anus
1st degree
treatment: high fbre/bulk diet, sitz baths, steroid cream (short course), pramoxine
(Anusol®), phlebotonics, rubber band ligation, sclerotherapy, and photocoagulation
Hemorrhoids degree:
bleed, prolapse, and require manual reduction
3rd degree
treatment: same as 2nd degree, but may require closed hemorrhoidectomy
hemorrhoid degree and management
bleed, permanently prolapsed, and cannot be manually reduced
4th degree
closed hemorrhoidectomy
surgical decompression is considered for thrombosed hemorrhoids for how many hours

managment fistula in ano that is high lying and involves external sphincter
staged fstulotomy with Seton
MGT: low lying fstula (does not involve external sphincter)
primary fstulotomy
MNGT: access fstula between sphincter muscles
■ ligation of intersphincteric fstula tract (LIFT)
chronic fissure triad
fssure, sentinel skin tags, and hypertrophied papillae
MNGT pharmacologic: Chronic fissure
topical nitroglycerin or calcium channel blocker (nifedipine or diltiazem), : botulinum toxin A
Most effective MNGT: fissure
lateral internal anal sphincterotomy (most efective)
