anal cancer etiology
A
HPV16 mediated, increased with HIV (40x), also smoking, fistulas/IBD. can be decreased with HPV quadrivalent vaccine.

poor prognosticators of anal cancer
A
> 5cm tumor size, or LN involvement (LN is worse prognosticator)

local excision for anal cancer
A
incidental resection at time of hemorrhoidectomy, or small well-diff tumors

anal cancer therapy
A
Nigril protocol- mitomycinC + 5-FU + RT. 65% 5-yr OS.

therapy if mitomycin is contraindicated
A
can give cisplatin-5-FU-RT instead.

local relapse of anal ca
A
can try salvage radiation.

chemo for metastatic anal ca
A
5-FU, mitomani, cisplatin, taxol, cetuximab (palliative)
