Gentamicin
Anti-pseudomonal
Topically for wound/burn infections
Aminoglycosides (group)
MOA: Inhibit protein synthesis-bind to 30s subunit (block initiation, prevent further translation/promote premature termination, promotes incorrect AA incorporation).
Uses: Severe systemic infections w/Gram neg
Coverage: Gram neg enterics, aerobes only (require O2 for transport into cell), bactericidal
Kinetics: IV only, does not penetrate CNS, kidney excretion
Can be combined w/penicillins but not in same syringe
AE: Serious ototoxicity and nephrotoxicity

Pre-op suppression of enteric aerobes and control severe diarrhea from E. coli
Tx of hepatic encephalopathy

Paromomycin
Intestinal amebiasis
Spectinomycin
Not available in US
Tx of uncomplicated gonorrhea by single IM injection, for pts allergic to cetriaxone or b-lactams
CI: don’t use for tx of pharyngeal gonorrhea
