System Definitions, Procedure Order Classes screen, list the subset of order classes from ORD-60 that apply to procedures.

No. It is assumed.
What impact does Order Type have on orders?
determines what tab the order will file to in Chart Review.
Where are the values for Order Type defined?

Would you typically specify the Order Type at the procedure level? Why/why not?
No. While you CAN set it in the Override Order Type field, it makes more sense to establish this at procedure category level to cover an entire group of similar procedures.
Is it better to set up default values for orders at the procedure level or the procedure category level? Why?
Defaults should be set up in the procedure categories as much as possible for ease of maintenance.
