Causes of Airway Obstruction?
CNS depression Blood Vomitus Foreign Body Direct Trauma Epiglottitis Pharyngeal swelling, infection/oedema Laryngospasm Broncos past Bronchial secretions Blocked tracheostomy or laryngectomy
Causes of VF
ACS Hypertensive heart disease Drugs: antiarrythmics, tricyclics, digoxin Inherited heart diseases: eg. Long QT Acidosis Abnormal K+, Mg2+, Ca2+ Hypothermia Electrocution
Airway Obstruction causes
CNS depression Inflammation Broncospasm Laryngospasm Blocked trach
Foreign body
Direct trauma
Infection
Recognition of airway obstruction
Can't talk Noisy breathing(stridor, wheeze) Choking Silent chest Paradoxical chest movements Splinting Use of accessory muscles
Treatment of airway obstruction
High flow obstruction
How to open airway?
Simple, adjuncts, advanced?
Head tilt
Chin lift
Jaw thrust (lift soft tissues away from airway)
Oropharyngeal device
Supraglottic airway device, tracheal tube
Causes of breathing problems
CNS Resp:
- drive
- effort
- pain
Lung disorders: asthma, COPD, embolus, oedema, pneumonia, haemothorax
Recognition of breathing problems
Look
Listen
Feel: expansion, tracheal position
Treatment of breathing problems
High flow oxygen guided by Sp02 (94-98%)
- support breathing: bag, mask
- treat underlying cause
Circulation problems
Pump
-structure, valvular, electrical
Bucket-hole
Bucket - became larger
Zipzag approach for 4 Hs and 4Ts
Hypoxia
Hyperkalaemia-metabolic
Hypothermia
Hypovolaemia
Tamponade
Thrombus
Toxins
Tension pneumo
Circulation approach
Syncope
SOB
Poor peripheral perfusion, temp, cap refill
Pulse-rate/regularity/volume
