Angiooedema effect on mast cell typtase?
It does not cause a rise
When does ACEi induced angiooedema usually occur?
Usually within one week but can be as long as 1.8 YEARS!
Pts with heredity angio oedema are at increased risk of which AI disease?
SLE
Note: If treatment fails to normalise the C4 levels and they remain persistently low; these patients are at an increased risk of developing SLE
Risk factors for allopurinol hypersensitivity syndrome?
Renal impairment
thiazide diuretic use
recent initiation of allopurinol (weeks/months).
Note: features are renal and hepatic dysfunction, eosinophilia, vasculitis and mortality is 20-25%
What is Jarisch-Herxheimer reaction?
Acute febrile illness with headache, myalgia, chills and rigors starting within 12 hours of the first dose of treatment (usually for syphilis) and resolving within 24 hours.
