Describe the mechanism of a true allergy? (may need to explain this to a px in regards to explaining allergy and anaphylaxis and use why we use EpiPen)

A
  1. Exposure/ sensitisation
    - the allergen is exposed to plasma b cells which produce IgE antibodies in response to the antigens
    - the IgE antibodies bind to IgE receptors on basophils and mast cells
  2. Re - exposure
    - the antigen bind to IgE antibodies on the basophils and mast cells when re exposed to the body again - thus results in degranulation of the mast cells and basophils and thus the subsequent release of histamine!

What are the effects of histamine on the body?

A
  • ๐Ÿ˜ฉ urticaria rash and itch
  • ๐Ÿซ smooth muscle contraction- hence we see bronchoconstriction and SOB and chest tightness in anaphylaxis
  • ๐Ÿ›Œ vasodilation - hypotension and angiodema
  • ๐Ÿ’ฆ increased leakiness of the capillary walls resulting in oedema and angiodema
  • ๐Ÿฉธ circulatory collapse - anaphylaxis

  • Penicillin is one of the narrow spectrum Abx that is used for various gram +ve infections thus being labelled PenA put u at risk for AMR and reduced choice

What is anaphylaxis?

A

A severe allergic reaction where the bodyโ€™s immune system becomes over reactive / hyper reactive when exposed to an allergen VERY QUICKLY!!!

โฐ it usually occurs within minutes of exposure to the allergen

It can be life threatening or FATAL

  • โš ๏ธ anaphylaxis is a medical emergency

A

We need to be aware of any problems with the patients airways, breathing, circulatation.

AND skin and mucosal changes - ie angiodema, urticaria and flushing

Why do we administer adrenaline in anaphylaxis, what are the effects on the body?

A
  • vasoconstriction - thus aims to increase BP and HR
  • reduces leakiness of capillaries - thus reduces oedema and swelling
  • smooth muscle relaxation - opens up the airways

  • anterolateral aspect of the middle thigh

A

NO - urticaria can even present in times of stress
When paired with other symptoms of allergy advice px to take antihistamines, if it doesnt calm down or px gets anaphylactic symptoms - ADRENALINE AUTOINJECTORS!!

General px counselling for adrenaline autoinjectors
- avoiding allergens is the best prevention of anaphylaxis - they are prescribed by brand name - ie Jext, EPi - pen - very important that you stick to one brand - get close family, work etc to be trained on how to use the specific autoinjectors - periodically check the viewing window and expiry date - if yellow (due to rx with sun โ˜€๏ธ), or out od date. - get a new one from GP or pharmacy - store at room temp - always keep TWO INDATE auto - injectors with you at **ALL** times - blue to sky - orange to thigh - no effect after 5 mins - 2nd injection into other leg, or or different spot - call 999 - anaphylaxis - medical emergency - faster response - swinging motion admin - hold for 10 elephants after you hear the click. Then rub area for 10 elephants.
