definition
= pulmonary hypersensitivity disorder recognized with increased frequency in USA
causes
by allergy to antigens of Aspergillus (species that colonize the tracheobronchial tree and usually occurs in atopic asthmatic individuals who are 20-40Y)
Primary criteria for diagnosis
- clinical history of asthma,
- periphereal eosinophilia,
- immediate skin reactivity to Aspergillus antigen,
- precipitating antibodies to Aspergillus antigen,
- elevated serum IgE levels,
- pulmonary infiltrates
- central bronchiectasis
secondary diagnostic criteria
- > Identification of Aspergillum in the sputum
- > Late- skin reactivity to Aspergillus antigen
ttt of choice
corticosteroids with excellent response.
prednisone 0,5 mg/kg/d given as a single morning dose for several weeks before transition to alternate day dosing.
several months of prednisone therapy may be required
complications
hemoptysis, severe bronchiectasis and pulmonary fibrosis

Amphotericin B
