Meningitis and +60 or immunocompromised
add amoxicillin IV 2g 4hrly
Encephalitis
IV aciclovir 10mg/kg tds

Amoxicillin 1g tds
pa: doxycycline PO 200mg day 1 then 100mg od

Co-amoxiclav 1.2g tds + doxycycline PO 100mg bd
pa: levofloxacin IV 500mg bd
7 days
CAP ICU/HDU/NBM
Co-amoxiclav IV 1.2g tds
+ clarithromycin IV 500mg bd
pa: levofloxacin IV 500mg bd
7 days
HAP non severe
PO amoxicillin
pa: doxycycline 100mg
5 days
HAP severe
IV amoxicillin + gentamicin
pa: IV co-trimoxazole + gentamicin
Step down: PO co-trimoxazole
7 days
Aspiration pneumonia non severe
PO Amoxicillin + metronidazole
pa: doxycycline 100mg bd + metronidazole
5 days
Aspiration pneumonia severe
IV amoxicillin + metronidazole + gentamicin
pa: replace amox with PO doxycycline or IV clarithromycin
Step down: PO amoxicillin + metronidazole
pa: doxycycline 100mg bd + metronidazole

Acute exacerbation COPD
Amoxicillin 500mg tds
or doxycycline 200mg on day 1 then 100mg od

Endocarditis native valve su bacute
Amoxicillin IV 2g 4hrly + gentamicin 1mg/kg bd
Endocarditis native valve severe sepsis
Flucloxacillin IV 2g 6hrly (or 4hrly if >85kg)
