Any home or establishment offering, coordinating or providing services to two or more persons who:

1. are domiciled therein
2. are unrelated to the operator
3. by choice or functional impairments, need assistance with personal care or nursing supervision
4. may need intermittent or unscheduled nursing care
5. may need medication assistance
6. may need assistance with transfer and/or ambulation

a home, establishment or institution providing nursing facility services as defined in Section 1-1902 of this title and one or both of the following:

1. assisted living center services as defined in the Continuum of Care and Assisted Living Act and
2. adult day care center services as defined in Section 1-872 of this title

Who determines application fee? How much is it? What is fee based on?

State Board of Health develops sliding fee scale on to exceed $1,000 per application.

Scale based on bed capacity of continuum of care facilities or AL centers

License fee of $10 for each bed included in the maximum bed capacity at such facility/center.

What additional fee accompanies application fee and license fee when an adult daycare is involved?

If a continuum of care facility includes an adult day care component, an additional license fee is required. The fee is determined by Board but not to exceed $75

1. Residential care homes
2. Adult companion homes
3. Domiciliary Care Units operated by the Department of Veterans Affairs
4. Private residences of persons with developmental disabilities receiving services provided by the DDSD of the Department of Health
