What is pre-renal AKI?
AKI secondary to decreased renal plasma flow
Causes of pre-renal AKI?
Decreased ECF volume
Effective decreased ECF volume (decreased CO, interstitial shift)
Disruption of renal hemodynamics (NSAIDs, RAAS blockers)
Hepatorenal syndrome (vasoactive compounds)
History suggestive of AKI?
Diarrhea, diuretics, poor oral intake
Signs of ECF volume depletion?
Hypotension, tachycardia, orthostatic changes, low JVP
Signs of decreased effective circulating volume
CHF, liver failure
Lab findings suggestive of pre-renal AKI
Hemoconcentration (high Hb, albumin)
Low urinary flow (high serum urea:Cr because slow filtrate allows more urea to be reabsorbed)
BLAND urine sediment
LOW URINE SODIUM
What is renal AKI?
AKI due to injury/dysfunction of renal parenchyma
What is the most common cause of AKI?
Pre-renal
What is the second most common cause of AKI?
acute tubular necrosis (tubulo-interstitial renal AKI)
