- it is a integrated system intended to track people over time through programs

A
- Organized so that ideally people transition along the continuum as their care needs change
- This assumes a linear trajectory :rarely the case
- Care trajectories are often unpredictable and non linear

- the consequences of leaving older adults in the community too long can be quite severe resulting in caregiver burnout , additional out of pocket expenses for caregiving.
- and too early can be social isolation , decline in functioning and depression in the older adult
