What is the SBP aim for haemorrhagic hypovolaemia?
> 70mmHg
How much fluid is given for a pt with haemorrhagic hypovolaemia?
If SBP <70mmHg:
250mL Bolus Normal Saline IV TTR
review after each bolus.
Rpt 250mL bolus until SBP >70mmHg (MAX 2L).
Consult for further doses.
What is the target SBP for pts with anaphylaxis who are also hypotensive?
SBP > 90mmHg
When do we treat an isolated SCI with fluids?
When SBP < 90mmHg
What dose of normal saline is given to an isolated SCI?
If SBP < 90mmHg:
10mL/kg TTR
What is the dose of normal saline given to an anaphylaxis pt with hypotension?
If SBP < 90mmHg:
40mL/kg IV TTR
Consult for further doses.
If consult U/A rpt 20mL/kg TTR
When do you treat Sepsis with normal saline?
If SBP < 90mmHg
What is the dose for normal saline in sepsis?
If SBP < 90mmHg:
Risk of fluid OL : 500mL TTR
All other adults : 1000mL TTR
What is the target SBP for a traumatic head injury?
SBP > 120mmHg
What is the dose of normal saline for a traumatic head injury?
If SBP < 120mmHg:
40mL/kg IV TTR
If SBP remains < 100mmHg after initial 40mL/kg consult for further dose.
If consult U/A rpt. 20mL/kg TTR.
What is the dose for normal saline for Shock?
Inadequate / extremely poor perfusion:
Risk of fluid overload: 1000mL TTR
All other adults: 2000mL TTR
When do you give fluids regardless of BP?
- Diving related emergencies (decompression illness)
- Burns
- DKA/HHS
- Dehydration
Dose for normal saline for decompression illness?
If chest is clear:
1000mL normal saline over 15-20min.
Rpt @4h
Dose for Adult burns if > 15yo?
If TBSA is > 15%:
TBSA x KG = V (mL) over 2h
Dose for Adult burns if 12-15yo?
If TBSA is > 10%:
3 x TBSA x KG = V (mL) over 24h
Half given in first 8h
