A

ACTH level is low

-Adrenal glands not producing cortisol because they are not being stimulated by ACTH

How short synacthen test (ACTH stimulation test) is done?

A

Performed in morning, give synacthen (synthetic ACTH).

Blood cortisol is measured at baseline, 30mins and 60 mins post- administration

Will produce cortisol & the level should double at least

Failure for cortisol to rise = primary adrenal insufficiency

Replacement steroids titrated to clinical signs and electrolytes

A

A

Describes acute presentation of severe Addison’s = life threatening due to lack of steroid hormones

Presentation of Addisonian crisis?

A
  • Reduced consciousness
  • Hypotension
  • Hypoglycaemia
  • Hyponatraemia, hyperkalaemia
  • Patients can be very unwell

Management of Addisonian crises?

A

-Intensive monitoring
-Parenteral roids (IV hydrocortisone 100mg start then 100mg every 6 hours)
-IV fluid resus
Monitor electrolytes and fluids
