What is Cushings syndrome?
A
Chronic glucocorticoid excess
Loss of normal feedback mechanisms of HPA axis
Loss of circadian rhythm

What are the ACTH dependent causes of Cushing’s syndrome?
A
- ACTH-secreting pituitary adenoma (Cushing’s disease)
- Ectopic ACTH production
- Ectopic CRF production

What are the ACTH independent causes of Cushing’s syndrome?
A
- Iatrogenic (steroid use)
- Adrenal adenoma/cancer
- Adrenal nodular hyperplasia

What can cause an ectopic ACTH production?
A
Small cell lung cancer and carcinoid tumours

In an adrenal adenoma/cancer, what would a dexamethasone suppression test show?
A
Dexamethasone doesn’t suppress cortisol
(tumour is autonomous

Increased weight
