capsule -> zona glomerulosa -> zona fasciculata -> zona reticularis -> medulla
(zona fasciculata makes up 75% of cortex)
What causes weight changes in adrenals?
weight changes due to lipid depletion (causes: weight loss, or prolonged stress)
causes hypertrophy and hyperplasia of the cortical cells which can more than double the weight
What are 3 types of steroids produced by adrenal cortex?
glucocorticoids, mineralocorticoids, and sex steroids
What does the adrenal medulla make?
chromaffin cell synthesize and secrete catecholamines (epinephrine)
What part of adrenal cortex makes glucocorticoids?
zona fasciculata with small contribution from zona reticularis
What part of adrenal cortex makes mineralocorticoids (specifically aldosterone)?
zona glomerulosa
What part of adrenal cortex makes sex steroids (estrogens and androgens)?
zona reticularis
What are 3 hyperadrenal syndromes?
hypercortisolism (Cushing Syndrome- elevation in glucocorticoids)
hyperaldosteronism (hypertension and hypokalemia from sodium retention and potassium excretion
adrenogenital (virilizing syndromes)
What are 4 distinctive etiologies of Cushing syndrome?
- exogenous glucocorticoids (most common of ALL - iatrogenic)
endogenous Cushing syndrome
- primary hypothalamic-pituitary diseases - hypersecretion of ACTH (most common endogenous)
- hypersecretion of cortisol by adrenal adenoma, carcinoma, or nodular hyperplasia
- secretion of ectopic ACTH by nonendocrine neoplasm
When Cushing Syndrome is caused by pituitary tumor, it is called…?
Cushing’s disease
What’s the etiology of primary hypothalamic-pituatary diseases causing Cushing syndrome?
women> men, in 20s and 30s
What are the causes of primary hypothalamic-pituitary diseases?
- Usually an ACTH-producing microadenoma
- Corticotroph cell hyperplasia in the anterior pituitary
patients have variable degrees of bilateral nodular adrenal cortical hyperplasia
What are the lab levels for primary hypothalamic-pituitary diseases?
high cortisol and high ACTH
