Admission
The official entry into a healthcare setting.
Discharge
The official departure of a person from a healthcare center.
Transfer
Moving the person to a different healthcare setting or into a new room with agency.
How can you provide comfort for the person who is being admitted or transferred?
*Explain what and why.
*Don’t rush the person.
*Be sensitive to their needs.
-If the person is in pain, call a nurse. Stay with them until the nurse has arrived.
Where does the admission process usually start?
In the Admitting Room or the ER.
What should you collect when preparing the room?
Admission Kit / Bedpan & Urinal / IV Pole / Nursing Admission Kit (Personal Stuff List) / V/S Stuff / Patient gown / Washcloths / Items Requested by Nurse.
Over-bed table ; Stethoscope, B.P Cuff, thermometer ;- Admission form.
How should you make the bed when they arrive by stretcher?
Arrive by w/c or ambulatory?
For the stretcher, make a surgical bed & raise the bed for transfer from the stretcher.
W/C or Ambulatory ; Ensure Bed is Closed. / Lower the bed to comfort lvl as directed by the nurse. / Put call light on sheets. / Wash hands.
During the admission procedure, the nurse may ask you to:
Room Prep./ ID./ Privacy./ Introduce u/rmte./ Gown./ Help into bed/chair./ Admission form & V/S./ Orient family/person to area./ Mug./ Call light./ Label stuff./ Help unpack./
Guidelines for measuring weight and height.
Gown only, no shoes./ Void before./ Dry pad./ Same time/scale, before breakfast./
Set Height to Zero.

Weight, paper towels, Set height, take robe/shoes, help onto scale, Balance plates.
Height, Posture, lower till hit head, Ft/in.
How do you Measuring height in bed?
Supine, co-worker should place ruler at patients heels, pull a couple inches above head.
Why may the person need to move to a new room?
Change in condition / care needs, per request /
Roommates don’t get alone.
Who can give an order to discharge?
Only the doctor can order discharge.
What would you do if a person wants to leave AMA?
Provide them an AMA form to sign, explaining they are leaving against medical advice and chart everything that happened.
What is the CNA role in transfer or discharge?
Dress if needed, help pack, check off stuff list, Tell nurse person is ready for final visit.
What will the nurse do for a transfer or discharge?
General ; Gives meds, discharge instructions, valuables in safe, person sign belongings list.
In W/C ; Gets w/c & utility cart for belongings, help into w/c, help out of w/c & into car.
Ambulance ; Raise bed rails, call light in reach, wait for ambulance staff, raise bed to highest when ambulance staff arrive.
