Comorbidity in ADHD?
Bipolar - stabilize mood first
1st line: bupropion
2nd line: Modafanil, CBT
MDD:
prioritize depression if moderate-severe
Guanfacine side effects
sedation, somnolence, reduced BP, reduced HR, headaches
When is atomoxetine indicated for first line?
- non responsive to stimulant
- significant side effects with stims
- tic disorder
- epilepsy
- substance use/diversion
- comorbid anxiety

Long acting stims:
- Methylphenidate - Concetra, Biphentin
- Adderral, Vyvanse
CADDRA - Second line
Dexedrine
Methylphenidate short acting
Atomoxetine
Guanfecine

Children - adolscent - 5%
Adults - 2.5%
Heritability RF in ADHD?
Parents with ADHD - >50% risk child dx with ADHD
1st degree relative - 30-40% chance of ADHD
Neuroimaging changes in ADHD
decreased brain volume
reduced brain volume in cortical areas
delayed brain maturation - delays in peak cortical thickness
ADHD evaluation in chidlren and adolescents
