adenotonsillitis Flashcards
to understand the medical concept of the condition
the components of the waldeyers ring include
the nasopharyngeal tonsils superiorly, the lingual tonsils inferiorly, the tubal tonsils laterally and the palatine tonsils anterolaterally
location of the tonsils
nasopharyngeal-behind the nasal cavity at the midline of the nasopharynx
tubal- around the opening of the eustachian tube in the lateral wall of the nasopharynx
lingual-at the posterior third of the tongue
palatine tonsils- at the tonsillar bed of the lateral oropharynx wall
the tonsillar bed is bounded by
the palatoglossal arch anteriorly and the palatopharyngeal arch superiorly
the majority of the tonsils typically drain into the
deep cervical and jugulodigastric lymph nodes
what is tonsillitis
inflammation of the palatine tonsils which are typically referred to as just tonsils
most cases of bacterial tonsillitis are caused by
Group A beta hemolytic streptococcus
symptoms of acute tonsillitis
fever
sore throat
foul breath/halithosis
odynophagia
tender lymph nodes
dysphagia
airway obstruction in acute tonsillitis may manifest as
mouth breathing
snoring
sleep disordered breathing
nocturnal breathing pauses
sleep apnea
symptoms of a peritonsillar abscess
severe throat pain
fever
drooling
foul breath
trismus(difficulty opening the mouth)
altered voice quality
the ‘hot potato voice’ is widely recognized as a symptom of
a peritonsillar abscess
adenotonsillitis is a clinical diagnosis T/F
True
epidemiology of tonsillitis
tonsillitis is generally the result of an infection whether bacterial or viral. for viral cases ,the common virus include rhinovirus, rsv, adenovirus and coronavirus. others include ebv cmv hbA hiv etc
for bacterial cause GABHS is typically the most common but S.aureus S.pneumoniae H.influenzae have also been cultured.
the paradise criteria for recurrent tonsillitis includes
recurrent sore throat or URTI or tonsillitis for at least 7 episodes per year or at least 5 episodes in 2 successive years or at least 3 episodes in 3 successive years
indications for tonsillectomy include
suspected malignancy/actual malignancy
recurrent tonsillitis
recurrent peritonsillar abscess
tonsillitis with associated sleep apnea
kissing tonsils
