Q

At the age of 18, Harry develops an interest in electronic music and starts going out to clubs. He is introduced to a drug, sometimes in the form of a pill, sometimes a powder he dabs on his gums.

When he takes it, he finds his experience of the music becomes more intense, he feels immense feelings of affection towards those around him, and is able to stay up all night dancing.

He does find there are some down sides to this drug. He finds he is constantly moving his jaw during the night and tries to drink a lot of water, but his friends intervene and stop him, saying they know this is dangerous.

The next day he feels very tired and low in mood, with a lack of motivation.

What drug is Harry probably taking?

A

MDMA

3,4-methylenedioxymethamphetamine

Commonly called ecstasy, MD, Mandy, Molly, Pills, Crystal

Q

What is the danger with water and MDMA?

A

MDMA causes dry mouth and makes the user want to drink a lot of water, which can lead to hyponatraemia and death.

Leah Betts was a young lady who died from severe cerebral oedema after drinking too much water while taking Ecstasy.

9
Q

True or false: Ecstasy is associated with acute euphoria and a hard-hitting come down.

A

True

Ecstasy causes a build-up of serotonin in the synapses which leads to feelings of euphoria and love, but ultimately leads to neurotransmitter depletion which means there is less availability of these happy feeling the next day.

Q

While at college, Harry is still going to club nights and is introduced to another drug (first one was MDMA) that makes him alert, energised, and excitable.

He also finds he can take some when having to pull an ‘all-nighter’ with work because it helps him stay up and also increases his focus and concentration.

One of Harry’s friends who took the drug frequently ended up hearing voices and thinking he was in danger and had to be admitted to a mental health unit.

What drug is Harry probably taking?

A

Amphetamine

Speeds, Billy/Billy Whizz, Sulph, Base

13
Q

What is the mechanism of action of amphetamine?

A

Binds to transporter proteins for monoamines (such as dopamine, NA and 5HT) and is taken up into neurons. Once inside it disrupts monoamine storage vesicles, leading to build up inside the neuron. This reverses the transporter mechanism and releases the monoamines, leading to increased free dopamine and NA.

Q

After trying amphetamine, Harry is then introduced to a similar drug, but which his friend says will make him way higher, way quicker. It looks like crystals and they smoke it. He finds his friend is right and quickly finds himself using it frequently.

Which drug has Harry discovered?

A

Methamphetamine

Meth, Yaba, Christine, Tina, Ice, Glass, Crystal Meth, Crank

16
Q

What is the mechanism of action of methamphetamine?

A

Very similar to amphetamine, but extra methyl group makes it more lipophilic; means it more readily crosses the blood-brain barrier and does the job faster and stays longer.

17
Q

What class of drug is methamphetamine?

A

Class B if swallowed, snorted, smoked

Class A if injected

18
Q

When on nights out, Harry also frequently takes a drug that he snorts as a white powder.

It makes him feel confident, energised, and excitable and finds chatting to people really easy. Although sometimes he can get agitated and aggressive instead.

Like the meth, he finds himself using it more and more frequently.

What is he likely taking now?

A

Cocaine

Coke, Crack, Blow, Charlie, White…and many many more

19
Q

What legal class is cocaine?

A

Class A

20
Q

What is the mechanism of action of cocaine?

A

Largely unknown, but increases the amount of monoamines available (dopamine, NA, 5HT)

21
Q

Sometimes, when Harry is having a really bad comedown on a Sunday, he gets really anxious and even sometimes has panic attacks.

He finds that there is a drug he can take, in the form of a pill, that calms him down and makes him feel relaxed and drowsy.

What is he likely taking?

A

Benzodiazepines

Benzos, valium, roofies

22
Q

What legal class are benzodiazepines?

A

Class C

23
Q

What is the mechanism of action of benzodiazepines?

A

Binds to GABA receptor, increasing effect of GABA (increases chloride in post-synaptic neuron, makes neuron less excitable)

24
Q

Harry finds himself doing a packing job in a factory on the weekdays, but really he is just “living for the weekend”, when he can party. He is deeply unhappy apart from when he is taking drugs, which feels like the only escape from a painful and meaningless life.

One Friday night, his friend introduces him to another drug, which is a brown powder that he initially smokes. Harry feels that, more than ever before, all the psychological pain he has been feeling the past few years goes away when he takes this drug. It is like an instant release from all his suffering.

What drug has Harry started using?

A

Heroin

Smack, skag, horse, brown, gear

25
Q

What legal class is heroin?

A

Class A

26
Q

How is heroin taken?

A

Smoked, snorted, injected

27
Q

What is the mechanism of action of heroin?

A

Crosses blood brain barrier and is converted to morphine, whereby it binds to mu-opioid receptors - reduces excitability of neurons, reduces GABA (an inhibitory neurotransmitter, therefore increased dopamine- high)

28
Q

How much heroin is a person using is they are spending £50/day?

A

~2-3 wraps per day

29
Q

What scale is used to classify a person’s opiate withdrawal symptoms?

A

COWS

Clinical Opiate Withdrawal Scale

30
Q

How does ICD-10 classify dependence?

A

3 or more of the following in the last 12 months:

  • Tolerance
  • Withdrawal
  • Using more for longer than intended
  • Unsuccessful attempts to reduce use
  • Large amount of time spent seeking, using and recovering from use
  • Drug use takes priority over social, work and recreational activities
  • Persistent use despite harm
31
Q

What is the half-life of methadone?

A

24hrs+

32
Q

True or false: Methadone’s half-life can reduce from 24hrs to 8hrs in pregnancy.

A

True
