ischemic stroke - most common
subarachnoid hemorrhage - high mortality
intracerebral hemorrhage
signs of acute stroke
SUDDEN onset of symptoms:
- numbness/weakness in face, limbs
- blindness in eye(s)
- confusion, trouble speaking/understanding
- trouble walking, dizziness, loss of balance/coordination
risk factors of acute stroke*
- hypertension - impt!!
- DM
- smoking
- familial hyperlipidemia
hypertension causing stroke
increase risk by 4 times
present in 70% of stroke pts
DM causing stroke
associated w/ htn
worser prognosis also
smoking causing stroke
increase risk by 3.5 times
especially in women
hyperlipidemia
high cholesterol levels -> increase risk in coronary artery disease -> stroke
antiplatelet used in what situation
ischemic stroke
have to take note of stroke mimicry caused by bleeding - cause if give antiplatelets will end up with more bleeding
treatment of acute stroke
antiplatelet brain scanning (CT/MRI) WITHIN 24 HOURS) - treat the HTN - neurosurgical assessment of pts w/ large cerebral stroke, acute hydrocephalus, cerebral hemorrhage - treat fever - glycemic control
inclusion criteria for rTPA (recombinant tissue plasminogen activators treatment)
unenhanced head CT does not show hemorrhage
given within 4.5hrs of onset
CT early signs of acute MCA infarct
- hypodensity in brain tissue
- obscuration of lentiform nucleus (blurs)
- swelling
- insular ribbon
- dense MCA
