Acute limb ischemia Flashcards
6 Ps:
pain (M)
paresthesia then anesthesia (M)
pallor (M)
paralysis (F)
pulselessness (F)
perishing cold (F)
Causes:
1) non traumatic
2) traumatic
traumatic:
direct injury
indirect injury: compartment syndrome, crushed injuries
non traumatic:
MCC: embolism, thrombosis
causes of embolism:
AF
post-MI
vegetations
rheumatic heart disease
atrial myxoma
prosthetic heart valve
acute emboli affects which part of the body more?
leg >arms
usually artery bifurcation
risk factors
smoking
DM
HTN
cholesterol
what should be asked in the past medical history?
TIA
Stroke
MI
possible examination findings
irregular pulse
valve clicks
murmurs
pulses in affected and contralateral limb
if pulse is present in the contralateral limb then the source is
Embolic
if pulse is absent in the contralateral limb then the source is
Thrombosis
investigations
FBC
U&E
CK
coagulation screen
urine dip (myoglobin)
CXR
ECG
cross match
cardiac/abdomen U/S
if thrombosis -> angiography
thrombosis-especial features
previous h/o claudication
muscle wasting
hair loss
ulceration
management
- analgesia - IV morphine
- IV fluids
- revascularization
a. embolectomy
b. thrombolysis +/- reconstructive surgery
revascularization time limit to prevent permanent muscle necrosis and metabolic effects
within 6 hours
