What investigations should be ordered in assessment of AKI?
Creatinine, urea, calcium, CK, protein electrophoresis (if appropriate), MSU microscopy, renal tract US
What is the use of normal saline associated with?
hyperchloraemic metabolic acidosis
What is I5HP?
a short interfering RNA which reduces p53 in renal tubular cells - shown to reduce AKI after cardiac surgery
What are the principles of treatment of AKI?
remove causes/exacerbating factors, manage fluids, mange hyperkalaemia/acidosis, RRT as required, stress ulcer prophylaxis, modify medication dosing
How can contrast induced AKI be prevented?
pre hydration, minimise contrast volume, discontinue nephrotoxins
What is the treatment for hepatorenal syndrome?
terlipressin
What is the treatment for rhabdomyolysis?
early aggressive fluids, ?IV bicarb, ?mannitol
