- unstable angina
- ST elevation MI

Acute Coronary Syndrome - symptoms
- Chest pain or discomfort
- Pain or discomfort in one or both arms, the jaw, neck, back or stomach
- Shortness of breath
- Feeling dizzy or lightheaded
- Nausea
- Sweating
Acute Coronary Syndrome - causes of a STEMI
- ST elevation
- new left bundle branch block
Acute Coronary Syndrome - no ST elevation
troponin blood test
- raised troponin –> NSTEMI
- normal troponin –> unstable angina (if no changes on ECG)

no typical chest pain

Acute Coronary Syndrome - Investigations
- physical exam, ECG, FBC, lipid profile
- Chest X-ray (PO)
- ECG (functional damage)
- CT coronary angiogram (coronary disease)
Acute Coronary Syndrome - acute STEMI treatment
- primary percutaneous coronary intervention (if available within 2 hours of presentation)
- thrombolysis (if PCI is not available within 2 hours)

- catheter into the brachial or femoral artery
- under x-ray guidance
use stent or balloons

- inject a fibrinolytic (break down fibrin)
- eg. streptokinase

- B - beta blockers
- A - aspirin 300mg
- T - ticagrelor 180mg (clopidogrel 300mg alt)
- M - morphine
- A - anticoag. eg. fondaparinux
- N - nitrates eg GTN
