What is Acne Vulgaris
A disease of the pill-debaceous units
Forms Of acne from mild-severe
Comedones (open or closed) - papule - pustules - nodules - cysts - scars
What is the peak age for developing acne
15-18 (90%)
Who can have a delayed onset
Females
Who gets acne more
Males=Females but males acne is more severe
Topical Cream (not antibiotic)
Benzoyl peroxide
topical antibiotics
Clindamycin and erythromycin
Systemic antibiotics
tetracyclines, erythromycin and trimethoprim
Last resort/best medicine
Isotretinoin - best results but saved as last resort
Side effects of isotretinoin
Teratogenic - must be on contraceptive
Low mood
Dry skin, lips, epistaxis, reduced night time vision.
Acne fulminans
Sudden onset acne eruption. Causes fever and unwell - start with isotretinoin and cover with prednisolone
Scarring treatment
Must wait 1 year after stopping isotretinoin
Can be steroids, excision, laser, chemical peels, dermabrasion (often have to go privately)
What is acne rosacea
Chronic inflammation ‘inflammation of the face without sweating’
Where is acne rosacea seen
Ace of clubs distribution - forehead, cheeks, chin, nose (unusual on non facial sites)
Who gets acne rosacea
F>M but (M more severe)
30-50
fair skin
Triggers of acne rosacea
alcohol, spices, emotion, hot drinks
What is rhinopehyma
thickening of sebaceous glands on the nose - after years of acne rosacea (enlarged nose tip)
Topical treatment
metronidazole, azeleic acid (only lasts 8 hours)
Common side effect
Rebound after medication wears off is very common - acne rosacea will appear worse
Other treatments
Oral antibiotics or low dose isotretinoin, light based treatments, laser
Childs acne treatment
has the same treatment as adults
Menstrual cycle acne
Treated with contraceptive (MUST have estrogen, progesterone only will make acne worse)
What is hidradenitis suppuritiva
Recurrent boils and abscesses in armpits, groin regions - treated with lifestyle modification and topical creams.
