DEFIBRILLATION
Monophasic (LifePak):
200J, 300J, 360J
Biphasic (Zoll):
120J, 150J, 200J
GLUCOSE
1 Amp D50 (2cc/kg)
*Can use for children >8 as well
EPINEPHRINE
1 mg IV
DILTIAZEM
0.25mg/kg IV (20mg Max) over 2 minutes
Followed by 0.35 mg/kg (25 mg Max) over 5 min
MAGNESIUM
2-4 G IV
AMIODARONE
(Pulseless)
300mg IV (push)
2nd dosing if no conversion: 150mg IV (push)
then infusion of 1mg/min for 6 hrs (360mg IV)
followed by infusion .5mg/min for 18 hrs (540mg IV)
**Don’t forget that vials in crash cart may be only 150mg dose, will need 2 vials
NARCAN
0.5-2mg IV/IM
ADENOSINE
6mg IV, rapid push
Repeat Dose: 12 mg IV, rapid push x2 (if needed)
PROCAINAMIDE
17mg/kg IV at 20mg/min - 30 mg/min
AMIODARONE
(VTach w/ Pulse)
Maintenance Dose?
150mg IV (infusion over 10 min)
Maintenance: 1mg/min for first 6hours,
then 0.5mg/min for next 18 hours
CALCIUM CHLORIDE (CaCl2)
20mg/kg slowly
*3x more potent than Ca Gluconate
**ONLY give through Cental Line**
***If given in PIV - may cause soft tissue/skin necrosis at the site
VASOPRESSIN
40 U IV
CARDIOVERSION
Synchronized
Narrow Regular: 50-100 J
Narrow Irregular: 120-200 J biphasic or 200 J monophasic
Wide Regular: 100 J
**Wide Irregular: DEFIB! (NOT synchronized)
ATROPINE
(Bradycardic)
0.5mg IV
INSULIN
Bolus: 0.1U/kg (10U) IV (not routinely used in adults)
Drip Rate: 0.1U/kg/hr
