- Tocolytic agent (halts labor)
- Do not use over 48-72 - black box warning, potential for serious maternal heart problems/death
- 0.25 mg SQ
- Will cause tachycardia in mom & baby-therapeutic
- May repeat
Mag Sulfate
- Calcium agonist (also used in preeclampsia)
- Given for neuroprotection prior to 32 weeks (prevents fetal brain injuries)
- Relaxes smooth muscles like the uterus
- Therapeutic serum level of magnesium 4-7 meq/L (5-8 mg/dL)
Betamethasone
- If pregnancy <34 weeks give two doses of 12 mg IM 23 hours apart, second dose can be given at 12 hours if delivery is imminent
- Last 7 days
- Can interfere with glucose regulation
- Help with formation of lung surfactant to reduce respiratory distress syndrome (accelerates process)
Nifedipine
- PO dose - order may say hold if BP <90/50
- Calcium channel blocker
- Main benefit of tocolytic drug is prolongation of pregnancy for 48-hour period to allow the steroid effect to enhance fetal lung maturity
- Cannot be given with magnesium sulfate
- Must wait 6 hours after last dose of Nifedipine to initiate magnesium sulfate
