What are the six major causes of accidents at work?
Pre-occupation Ignorance or lack of skill Over familiarity Laziness or fatigue Physical hazards Horseplay
Pre-Occupation
This can be caused by personnel worries and/or distractions such as problems at home, illness, upcoming leave/holiday.
Ignorance or lack of skill
Ignorance is not knowing something,
Lack of skill is simply inexperience.
Over familiarity
Instructors fall under this category. We have done tasks so many times that we often take things for granted.
We may perform a task without a checklist at hand unaware (ignorant) that a change in procedure has occurred.
Laziness or fatigue
Taking shortcuts, shortcuts are not necessarily bad. But there are times when the sort cut will not work. Being in a hurry and doing things in a dangerous manner.
Physical hazards
Oil on the deck
Unguarded machinery, moving machinery
Toxic processes, toxic substances and components.
Hatches and guard rails.
Horseplay
This is nothing more than just messing around and not taking into account your the task or your surroundings.
Preventing accidents.

Maintain safety discipline
Develop the habit of noticing potential dangers
Know the hazard.
How can you help to identify hazards that have already been recorded.
Review the risk assessment on file to familiarise yourself with all of the hazards/dangers involved with the tasks or job your doing.

Most tasks have procedures you follow to minimise the risk. This falls under using common sense by doing the job properly, follow the instructions.
