Community acquired pneumonia: organisms and drugs

A

Organisms: Strep pneumoniae (resp G+), H. influenza (resp G-),
atypicals: M. pneumoniae, C. pneumonia, legionella, P. aeruginosa, CA-MRSA

Drugs:

  • Outpatient, no comorbidities: Macrolide (Azithromycin), Doxycycline
  • Outpatient, comorbidities: FQ, Beta lactam plus macrolide
  • Inpatients/ non-ICU: Resp FQ (1st line), Beta lactam plus macrolide, or any other combo of cell wall synthesis inhibitor plus protein synthesis inhibitor
  • -if p. aeruginosa: antipseud. B-lactam (piperacillin/tazobactam, ceftazidime, cefepime, impenem) + resp FQ
  • -if CA-MRSA: add vanco or linezolid
