Organisms: Group A Strep, Strep pyogenes
Drugs: Penicillin V (PO) or G, Amoxicillin (PO)
Rhinosinusitis: organisms and drugs
Organisms: S. pneumoniae, H. influenza, M. catarrhalis, S. aureus
Drugs: Augmentin, doxy, clinda + 3rd gen cephalosporin, resp FQ
Otitis media: organisms and drugs
Organisms: S. pneumoniae, M. catarrhalis, H. influenza, Respiratory viruses
Drugs: Amoxicillin or amoxicillin clavulanate, azithromycin, 3rd gen ceph
Community acquired pneumonia: organisms and drugs
Organisms: Strep pneumoniae (resp G+), H. influenza (resp G-),
atypicals: M. pneumoniae, C. pneumonia, legionella, P. aeruginosa, CA-MRSA
Drugs:
- Outpatient, no comorbidities: Macrolide (Azithromycin), Doxycycline
- Outpatient, comorbidities: FQ, Beta lactam plus macrolide
- Inpatients/ non-ICU: Resp FQ (1st line), Beta lactam plus macrolide, or any other combo of cell wall synthesis inhibitor plus protein synthesis inhibitor
- -if p. aeruginosa: antipseud. B-lactam (piperacillin/tazobactam, ceftazidime, cefepime, impenem) + resp FQ
- -if CA-MRSA: add vanco or linezolid
urinary tract infection: organisms and drugs
Organisms: E. coli, proteius, k. pneumoniae, s. saprophyticus
Drugs:
- uncomplicated: nitrofuantoin, fosfomycin, TMP-SMX
- complicated: levoflox, ciproflox, cephalosporin, augmentin
Gonococcal infeciton: organisms and drugs
Organisms: N. Gonorrhoae
Drugs: Ceftriaxone (IM) or Cefixime (PO) PLUS Azithromycin (PO)
Syphilis: organisms and drugs
Organisms: Tremponema pallidum
Drugs:
- Primary or latent: DOC = PCN G or benz PCN G (IM), doxy, cephtriaxone
- Tertiary or neurosyphilis: PCN G (IV) or doxy (PO), or tetracycline
SSTI (cellulitis): organisms and drugs
Organisms: Group A strep, S. aureus
Drugs: Mupirocin (topical)
- If not responding to topical ABX:
a. Lenezolid (PO, IV)
b. TMP-SMX (PO, IV)
c. Doxy (PO)
d. Vanco (IV): DOC for MRSA, DOC if PCN allergy
e. Minocylcine (PO, IV)
f. Daptomycin (IV)
g. Dicloxacillin (PO)
h. Cephalexin (PO)
i. Clindamycin (PO, IV): DOC if PCN allergy
j. Nafcillin (IV): Parenteral DOC for MSSA
k. Cefazolin (IV)
l. Ceftaroline (IV)
