- used for at risk pts
- history or infection
- exposure to infection
- endocarditis prophylaxis
- pneumococcal prophylaxis

A
- prescribing abx based on knowledge of most likely organism
- educated guess
- make sure abx is indicated
- used when unable to wait until organism is known
- culture first when possible
- broad spectrum or combination therapy may be needed
