oral rehydration therapy
compenents of ORS: carbs (glucose), sodium, potassium, Cl- , water
takes advantage of glucose coupled sodium transport in small bowel –> enhances sodium and water transport across intestinal walls
dont use gatorade, fruit juice or soda - too high of gluocse, may increase diarrheal losses!
Ciprofloxacin
Fluoroquinolone
MOA: targets bacterial DNA gyrase - prevents relaxation of positive superboils
tx: E. coli, shigella, enterobacter, campylobacter (traveler’s diarrhea and shigellosis)
SE:
- GI nausea/vomiting
- CNS dizziness and hallucinations
- achille’s tendon rupture in children!
Levofloxacin
Fluoroquinolone

Trimethoprim- sulfamethoxazole
“bactrim”
bacteriostatic drug
use: shigellosis, traveler’s diarrhea
AE: allergic skin rash, nausea, vomiting, CNS problems
** drug interactions with warfarin!!
Metronidazole
“Flagyl” = antiprotozoal
tx of mild/moderate C. diff
AE: nausea, diarrhea, hypersensitivity reactions
don’t drink alcohol with this!!!!
Vancomycin
glycopeptide that inhibits cell wall synthesis by binding D ala
tx of severe C. diff
AE’s: otoxocitiy, nephrotoxicity , red-man syndrome
traveller’s diarrhea tx?
tx w/ loperamide/bismuth subsalicylate
or w/ FQ (Cipro or Levo)
C. diff tx?
Vancomycin (severe)
Metronidazole (Mild/moderate) - less expensive, don’t want DR to occur w/ Vanc
shigellosis tx?
do tx w/ Abs
tx of persistent diarrhea?
suspect giardia - tx w/ metronidazole
what do you not treat w/ Abs?
EHEC and Salmonella
