What is a Threatened Abortion?
Vaginal bleeding within the first 20 weeks of gestation. Not ending in spon abortion
No cervix dilation
No Expulsion of POC
What is a Inevitable Abortion?
Cervix Dilated but no tissue passage. Viable pregnancy unlikely
What is a Incomplete Abortion?
Cervix Dilated and some tissue passage but not all.
What is a Complete Abortion?
Vaginal Bleeding decreased
All POC expelled
Cervix Closed
What is a Missed Abortion?
Fetus died with or without spotting
No cervical dilation
Complete retention of POC
Will proceed to full abortion within 1-3 weeks
What is a Septic Abortion?
Thretened, inevitable, or incomplete abortion with signs of uterine infection
What are the signs of uterine infection?
Elevated Temp Leukocytosis Lower Abd tenderness Cervical Motion tenderness Foul Smelling Discharge
What are the causes of Abortion?
Chromosomal Abn 50% Congenital Anomaly -Teratogen -DM -Trauma -Anatomic Abn -Infection No obvious etiology for many
What are the risk facotrs for miscariage?
Inc maternal age
EtOH, Smoking, drug use
Recurrent Preggo Loss
Extremes of weight
How is a miscarriage diagnosed?
Vaginal bleeding and cramping Dec Signs of preggo Quant hCG level Transvaginal US Sterile Speculum exam Sterile vaginal exam Serum Progest
What is included in the DDx of Miscarriage?
Ectopic PReggo
Placenta prrevia
Physiologic
Cervical or Vaginal pathology, trauma or infection

RhoGAM
Prophylactic Abtx for Surgical Management
Emotional Support
OR: Suction D&C
What are aspects of post D&C Care?
Discharge home with Rx for Pain meds, Abtx and Uterotonic
follow up in 10-14 days
What is the Tx of Threatened Miscarriage?
Pelvic Rest
Min to no intervention
Follow up in 1 week
