What is the abortion?

What are the three determinant of the abortion?
Vaginal bleeding, cervix dilation, pain…
Define threatened abortion
Painless, vaginal bleeding without cervix dilation and expulsion of POC [Normal pregnancy with bleeding]
Incomplete and complete abortion, what is the difference between them?
Mention their signs…
Incomplete abortion partial expulsion of POC while Complete abortion is complete expulsion of POC before 20 WKs of Gestational age.
Signs:
1. Lower abdominal pain
2. Vaginal bleeding
3. Open cervical os
Define inevitable abortion…
Inevitable abortion: painful vaginal bleeding from retro placental site, the POC is about to aborted but not yet..
-Cervical Os is dilated
What is the Missed abortion?
Death of embryo or fetus before 20 weeks of Gestational age, with complete retention of POC.
- no vaginal bleeding
- closed cervix os
- decreased pregnancy symptoms
Recurrent abortion???
3 or more following continuously abortion
What is septic abortion?
Septic abortion: Infection that occur during abortion
Abortion complications can be acutely or chronicle, mention both of these complications
Acute complications:
1. Incomplete abortion
2. Hemorrhage
3. Sepsis
4. Uterine perforation
5. Bowel injury
Chronic complications:
1. Chronic pelvic pain
2. PID
3. tubule blockage [secondary infertility]
4. Ectopic pregnancy
5. Increased risk of recurrent abortions
What is the SAB, and mention most common causes of SAB?
SAB: expulsion of POC spontaneously with or without complete
Cause: chromosome abnormalities
List risk factors of SAB..
- Infection
- Maternal anatomic defects
- Immunological factors
- Environmental factors
- Endocrine factors
Most common SABs has no obvious causes
What are the symptoms of the 1st trimester abortion..
- Vaginal bleeding
- Cramping
- Abdominal pain
- Decreased symptoms of pregnancy
Diagnosis 1st trimester abortion..
- Physical examination [vital signs and febrile illness]
- Pelvic examination [uterine and cervix condition, sources of bleeding]
- Lab tests [CBC,b-HCG level, blood group..]
- Ultrasound
Differential diagnosis of 1st trimester abortion, mention..
- SAB
- Postcoital bleeding
- Ectopic pregnancy
- Vaginal or cervical lesions
- Extrusion of molar pregnancy
- Non pregnancy causes of bleeding
