Upper G.I. tract organs in upper quadrants Lower GI tract organs in all quadrants Remember 4 quadrants and 9 regions Localization of pain
RIght Upper Quadrant (RUQ)
- liver: right lobe
- gall bladder
- ascending colon, right colic flexure and right half of transverse colon
- stomach (pylorus), duodenum, head of pancreas
LUQ
- liver: left lobe
- most of stomach
- spleen
- transverse colon (left half), left colic flexure, descending colon
RLQ
- small intestines, appendix, cecum, lower part of ascending colon
- right side of full bladder or pregnant uterus
LLQ
- small intestines, lower part of descending and sigmoid colon
- left side of full bladder or pregnant uterus
Epigastric region (1/9)
- liver
- gallbladder
- stomach
- pyloric sphincter, superior duodenum, head of pancreas
REFERRED PAIN FOR DISEASED GALLBLADDER, STOMACH, DUODENUM, and PANCREAS occurs here
Umbilical region (1/9)
- lower duodenum, loops of small intestine
- parts of transverse colon
Hypogastric or Pubic region
- sigmoid colon
- ureter and upper bladder
Lumbar and Inguinal regions
-parts of ureters
Gallbladder (early referred + somatic pain from peritoneal irritation )
early referred pain - epigastric region
somatic pain:
- right side of epigastrum
- near the costal margin
- right line semilunaris
Murphy’s sign - have patient inhale and press on skin above their gallbladder. They will stop inhaling if there is pain.
Pyloric area of stomach and first part of duodenum pain due to perforated ulcer or gastritis
early referred pain - epigastric region
somatic pain - right edge of epigsatrium but can also extend to umbilical and hypogastric areas.
Appendix
early referred pain - umbilical area
after peritoneal irritation - lateral area of RLQ
McBurney’s point - location of appendix. Found 1/3 of the way between ASIS and umbilicus.
referred pain from secondary irritation of the diaphragm (organs + location)
Organs - gallbladder, pylorus of stomach, duodenum
Location - upper RIGHT shoulder and lower RIGHT cervical region
Cecum and sigmoid colon pain
Cecum - right side of hypogastrium
Sigmoid colon - left side of hypogastrium
Stone in renal pelvis
referred pain to posterior abdomen in area of costovertebral angles (TAP SMARTLY)
Stone movement from upper to lower ureter
inferolaterally from costovertebral angle to the posterior lumbar region and to anterior lumbar/inguinal regions
