Inspect the abdomen

A
  • Contour- could indicate a mass
  • Symmetry- should be symmetrical.
  • Umbilicus- Normally midline and inverted.
  • Skin- Surface is smooth and even. If patient has scars- note length, location.
  • Pulsations- Normally can see aorta pulsating in thin people.
  • Demeanor- Should be relaxed.

Bowel sounds

A
  • Listen to all 4 quadrants
  • Normal sounds are high pitched, gurgling, cascading sounds, occurring irregularly.
  • 5-30 gurgles per minute.
  • Use diaphragm with light pressure. Start in the RLQ.

Hyperactive bowel sounds

A
  • Loud, rushing, tinkling sounds that signal increased motility.
  • Borborygmus- “stomach growling”- fairly common

  • Listen for 5 min in each quadrant before you document them as silent.
  • Can follow after surgery

Vascular sounds

A
  • Listen for bruits in 7 areas.
  • Use diaphragm with firm pressure.
  • If you suspect you heard a bruit use the bell.
  • Note location, pitch and timing.

  • Percuss all four quadrants in a clockwise zig zag pattern.
  • Normal sound is Tympany.
