Example of vulnerable populations? Why are they vulnerable?
1.) Seniors - due to outdated information (not reading what’s new, not able to look on the internet), losing cognitive ability (decreased LOC), polypharmacy, not knowing who, what or where is a reliable source, sensory deficits (can they hear, see, speak as well as well as they should)
2.) Immigrants - due to language barriers, availability of what they’re used to (food, culture, health care, etc)
3.) Unemployed -due to the poverty line (social determinants of health) – finances, education, etc..
What interventions could we do that affect C?
-We can change the patient position, administer fluids, medications, administer blood
What is literacy?
The ability to read, speak, write, and understand in one’s native language
The average person is at a grade 5 literacy level
What is a digital immigrant?
Refers to a previous generation that has had to learn how to use technology Vs a digital “native” and how they grew up with the technology and how we are more digital literate
What are we looking for under E? How do we asses this area?
E = Exposure
Expose skin and observe for any signs of injuries (bleeding, bruising, trauma, rashes)
-We are looking or exploring here to see what could be causing the issue or what have our patients been exposed to!
Temperature is looked at here unless it is hypothermia!!
What are we looking for under D? How do we assess this area?
D = Disability
Level of consciousness, AVPU, GCS, pupils, blood glucose, limb movements (GCS is a big one)
*Look at blood glucose here
-We are looking here to see if there is evidence of an injury that I need to stabilize (fracture)
What are we looking for under A? How do we assess this?
A = airway
-We are assessing if their airway is intact or if they have a blockage
-Can we hear breath sounds? Can they move air in and out? Can they talk?
-Looking for presence of voice sounds and breath sounds
What are we looking for under C? How do we assess this?What do we need to properly assess this section?
C = circulation
We are looking at Skin color and temp, sweating, capillary refill time, heart rate and rhythm, heart sounds, BP, presence of any bleeding
-To assess we need BP cuff, stethoscope
What is Autonomous Competence?
It deals with personal empowerment and having the ability to judge and use information
-Patient is making their own decisions (They are speaking up and saying/telling you what they want)
-Patient is able to recognize and understand the actions from their decisions
What is health literacy?
Skills to enable access, understanding and use of information for health
These literacy skills are used for a wide range of daily tasks, such as making healthy lifestyle choices, finding and understanding health and safety information, and locating proper health services
What is the purpose of the ABCDE approach?
To provide life saving treatment
To break down complex clinical situations into more manageable parts
To serve as an assessment and treatment algorithm
To establish common situational awareness among all treatment providers
To buy time to establish a final diagnosis and treatment
What must we do/ensure a patient has in order to test and respect their autonomous competence?
We need to make sure that our patient also has the appropriate knowledge to make them fully autonomous
-We need to ensure/evaluate that our patient is also able to follow through
What are we looking for under B? How do we assess this?
B = Breathing
-This is where we are concerned if they are filling their lungs
-We are looking at their resp rate, equal chest wall movement, chest percussion and auscultation, O2 stats and requirements
What are the 4 centeral focuses on prioritization of client care?
Priority 1: Life-threatening illness
Priority 2: Safety
Priority 3: Client priorities
Priority 4: Nurse priorities
