Responsibilities:
Evaluating patients condition and needs.
Developing treatment plans to address a patients needs and help them meet specific goals.
Assessing a patients home/ and work environment and recommending adaptations to fit the patients needs and improve independence.
Training patients and their caregivers to use special equipment.
Assessing and documenting progress for evaluation, billing, and reporting purposes.
Care Manager
Care managers are professionals who provide comprehensive and coordinated care to individuals in need.
Responsibilities:
meet legislative requirements by putting in place policies and procedures.
