Section II - transport required
Abdominal pain
Suspected isolated Fx of the hip
Abnormal vaginal bleeding
Suspected allergic reaction
Asymptomatic exposure to haz mat (if known to have delayed symptoms)
GI Bleedimg
Near drowning
Danger to self, others or gravely disabled
Section III - Pediatric patients
Infant less than or equal to 12 months, transported regardless of c/c
Children 13-36 months of age require base hospital contact and/or transported except isolated minor extremity injury
If parent AMA, base contact required
Section IV - Required base contact criteria for SFTPs
If indicated on the SFTPs
For any criteria listed in section I that is not addressed by SFTPs
Whenever consultation with base hospital is indicated
Section I - Base Contact Required
S/S of shock
Cardiopulmonary arrest
Chest pain/discomfort
SOB/tachypnea
PMC/PTC
5 or more patients requiring transport
Contacting MAC
ALOC
Suspected ingestion of poison substance
Exposure to hazardous material with medical complaint
Abdominal pain pregnancy or suspected pregnancy
Childbirth or signs of labor
Suspected femur Fx
Facial, neck, electrical or extensive burns
- 20% or > in adult
- 15% or > in children
- 10% or > in infants
Trauma criteria
Traumatic crush syndrome
Syncope, LOC, acute neurological symptoms
AMA
