#8: Anaphylaxis Flashcards
Most critical concerning locations in angioedema: (3)
Tongue
Lips
Face
Hereditary angioedema MC affects these 2 locations:
Respiratory
GI tract
Hereditary angioedema MC cause and SXS duration:
Minor Trauma
Lasts hours to days (1-2 d)
Hereditary angioedema is an autosomal dominant dz characterized by these 2 lab abnormalities:
1- Low/dysfx. Levels of C1 esterase
2- low levels of C4 between acute attacks
Prophylaxis agents for acute attacks of angioedema: (2)
1- stanazolol (Winstrol)- 2 mg/day
2- danazolol (Danol)- 200 mg/day
Hereditary angioedema acute attacks are shortened with these 2 agents:
Fresh Frozen Plasma
Berinert (C1 esterase inhibitor)
SXS consistent with food allergies: (3)
- swelling and itching of the lips, mouth and pharynx
- n/v/d
- abdominal pain/cramps
- -> also lead to anaphylaxis, urticaria, or angioedema
Food allergy sensitivity TX: (3)
- MC: H1B
- - also, H2B and Steroids
Insect hypersensitivity rxn MC initial SXS: (4)
sxs of local, toxin-mediated rxn:
- pain
- pruritis
- swelling
- redness
- **DO NOT CONFUSE WITH CELLULITIS
Allergy Drug rxn risk is associated with this:
ability of the molecule to remain PRO bound (increased, then increased risk for rxn)
If pt has a sulfa allergy, they are likely allergic to all sulfa-based products: (8) ABCDG
- Abx (topical): silver sulfadiazine (silvadene) and mafenide acetate
- Bactrim
- Celecoxib
- Diuretics [TZD, loops- furosemide (lasix) and bumetanide (bumex)]
- Glucose regulation: diazoxide and glipizide
SXS consistent with a sulfa allergy: (4)
resembles serum sickness:
- arthralgia
- myalgia
- pruritis
- urticaria
MC med causes of anaphylaxis: (6)
- β-Lactam antibiotics (PCN MC overall)
- (Bactrim) TMP-SMX
- Vancomycin
- Vaccines
- NSAIDs
- Salicylates
MC food causes of anaphylaxis: (7)
Shellfish Eggs Nuts (peanuts and tree nuts) Dairy Soybeans Or Wheat Seeds
MC environmental causes of anaphylaxis: (5)
- Radiographic contrast
- Insect particles/hymenoptera stings (2nd MC overall)
- Molds
- Latex
