67. HPV and Polyomavirus Flashcards
1
Q
human polyoma virus epi?
A
- seroprevalence peaks in kids 5-9
- immunity wanes over time (81% seropositivity)
- respiratory transmission
2
Q
human polyomavirus structure?
A
- early coding region w/ T(transformational) proteins that bind to and inactivate p53 and Rb ptoein
assembled in nucleus, non-enveloped, smaller than HPV
3
Q
polyomavirus life cycle?
A
- JCV infects oligodendrocytes, astrocytes (lytic infection, myelin destruction)
- sxs usu subacute but incl discoordination, gait imbalance, cognitive dysfunction, vision probs
- occurs in immunosuppressed (HIV, cancer, transplant, mAB tx)
4
Q
polyomavirus clinical spectrum?
A
- primary infection usu asymptomatic
- disease in immunocompromised
- JCV = progressive multifocal leukoencephalopathy (PML) – CNS disease
- BKV = polyomavirus-assoc nephropathy (renal transplant), uretal stenosis & hemorrhagic cystitis – transplant patients
