4.5 Embolism Flashcards
1
Q
Types of embolism (5)
A
- Thromboembolus (>95%)
- atherosclerotic embolus
- fat embolus
- gas embolus
- amniotic fluid embolus
2
Q
atherosclerotic embolus
-how to identify in histology?
A
- plaque that dislodges
- characterized by presence of cholesterol clefts in histology
3
Q
fat embolus
- associated with what problems
- how to identify in histology?
A
- associated with long bone fractures and soft tissue trauma
- fat often enters pulmonary vessels, causing dyspnea
- histology: large fat globules in vessel lumen.
4
Q
gas embolus
-associated disorder
A
- Decompression sickness–“bends” (joint, muscle pain) and “chokes” (resp symptoms)
- bubbles of nitrogen
5
Q
amniotic fluid embolus
A
- amniotic fluid, full of fetal skin cells, enters maternal circulation.
- SOB, CNS symptoms, DIC (amniotic fluid has tissue thromboplastin)
6
Q
why are most pulmonary embolisms clinical silent?
A
- lung has dual blood supply (pulm and bronchial arteries)
2. embolism too small (self-resolves)
7
Q
when does pulmonary embolism cause sudden death?
A
-requires large saddle embolism that blocks both pulm arteries at once. Heart cannot pump!
