o intrinsic connect individual carpal bones o extrinsic carpals to radius, ulna, metacarpals volar and dorsal groups
5 specific aspects to assess on wrist xray
- Radial length
- Ulnar slant
- 3 carpal arcs
- Volar tilt
- Pronator quadratus fat pad
What is normal radial length
radial styloid should extend 9-12mm beyond ulnar articular surface
What is normal ulnar slant
Ulnar slant of distal radius should be 15-25 degrees
What is normal volar tilt
10-25 deg in lateral
3 PHx to test for scaphoid #
- snuffbox
- axial compression
- Wrist pinch

CT
spica with ortho FU
Signs of lunate #
Pain in depression distal to listers tubercle
MGMT of lunate #
- Non displaced are immobilized and displaced are ORIF
Complication of lunate #
Keinbock disease
- AVN of lunate
MGMT of triquetral #
immobilize in short arm splint or ulnar gutter and ortho FU
Complication from pisiform #
ulnar nerve parathesia
MGMT of pisiform #
Short arm cast x 3 weeks
- If neuropraxia, then urgent ortho
PHx for hamate #
- Isolated pain over hypothenar eminence
MGMT of hamate #
- Hook #s should be immobilized with ulnar outrigger and thumb spica
- If distal ischemia, urgent ortho
MGMT of trapezium
spica with ortho follow-up
