Who gets sjogren’s syndrome?
MC - females 30-50 y/o
What is sjogrens syndrome?
Chronic inflammatory disorder mainly characterize by lymphocytic infiltration of exocrine glands
Most prominant symptoms of Sjogren’s syndrome?
Dry eyes (>3mo) and mouth
MC symptoms of Sjogren’s syndrome?
1st MC: K. Sicca or xerophthalmia
2nd MC: xerostomia
Also:
salivary gland enlargment, Upper airway dryness, vaginal dryness, dysparunia
What causes the symptoms of Sjogren’s syndrome?
Lymphocytic infiltration of glandular and nonglandular organs
Common symptoms of Sjogren’s syndrome?
Xerosis
- Dry eyes, mouth, mucous membranes, skin
Cutaneous leukocytoclastic vasculitis Raynaud’s Arthralgias Non-erosive arthritis (symmetric) Mild inflammatory myopathy Fatigue Neuro
MC complaint Sjogren’s syndrome?
Fatigue
MC neuro manifestation of Sjogren’s syndrome?
Peripheral neuropathy
Diagnostic studies for Sjogren’s syndrome?
Schirmer test
X ray
Salavary gland biopsy
Lab findings for Sjogren’s syndrome?
ANA: >80% RF: 40-50% Anti-Ro and anti-La (lupus) Hypergammaglobinemia (MM) Positive anti-SSA or anti-SSB
MC serologic findings for Sjogren’s syndrome?
- Cytopenia (normicytic anemai, leukopenia, thrombocytopenia)
- H ESR
- hypergammaglobulinemia (monoclonial IgG)
Sjogren’s syndrome has a high risk of?
Lymphoma (lymphandenopathy)
Tx for Sjogren’s syndrome?
Symptomatic only
Dry eyes: eye shit Dry mouth: - pilocarpine - cevimeline - lemon drops
Fatigue/arthralgis/myalgias: - plaquenil (like SLE) Moderate extraglandular: - prednisone Intra organ involvement: - prednisone and methotrexate
Limit prednisone and methotrexate to?
Sever cases
Sjogren’s syndrome, avoid?
Anticholinergic drugs
Tobacco
